What is ICSI?

Intracytoplasmic sperm injection, or ICSI, is a fertilisation technique used in IVF. It involves injecting a single healthy sperm into each mature egg to maximise the chance of fertilisation in couples where there is a male fertility problem.

As the sperm directly fertilises the eggs, issues with sperm motility (movement), sperm morphology (shape), or number (low sperm count) can be overcome, giving men with sperm problems the chance to become fathers. 

ICSI may also be recommended for:

  • Patients who have had failed fertilisation or very low fertilisation in previous treatments
  • Some cases of frozen sperm where the number of motile sperm are low
  • Surgically retrieved sperm
  • Cases where eggs have been frozen

What is different about Essential ICSI?

ICSI is the most common treatment for male fertility issues. The technique is the same for patients as a standard ICSI treatment, where the sperm is injected directly into the egg. The only difference is that it is a lower-cost treatment compared to standard ICSI as the consultations and treatment planning stages are nurse-led.

Senior Consultant Raj Mathur will oversee Essential ICSI procedures. Experienced fertility doctors and consultants will carry out egg collections and embryo transfers.

Who is Essential ICSI for?

  • Sperm Issues

    ICSI is recommended for issues with sperm, such as low sperm count, sperm morphology (shape) issues movement and sperm motility (movement) issues.
  • Failed Fertilisation

    ICSI can also been recommended for patients who have had failed fertilisation or very low fertilisation in previous treatments.
  • Frozen Sperm & Eggs

    ICSI can also be used in some cases of frozen sperm where the number of motile sperm are low or the sperm has been surgically retrieved or cases where eggs have been frozen.

Am I eligible for Essential ICSI?

In order to proceed with Essential ICSI, we ask patients to meet our criteria, which are set to ensure the best possible success rate for you. The key indicators for treatment are age, BMI and ovarian reserve.

  • Aged 37 or under

    We can offer treatment for women aged 37 or under.

  • BMI under 32

    We know a healthy BMI for fertility treatment is between 18 - 32.

  • AMH above 10

    Treatment works best with an ovarian reserve between 10 - 35.

  • Regular menstrual cycles

  • No underlying health conditions

Certain medical issues identified, including any anomalies identified on a scan, could mean that you are not able to proceed with Essential IVF/ICSI. ICSI Treatment Process

1Check Eligibility Criteria

Before we can begin your Essential ICSI journey, you will need to check your eligibility via our website or by calling our New Patient Coordinator team. If you need tests to see if you meet our eligibility criteria, your New Patient Coordinator will arrange these. Female tests include an AMH blood test and pelvic ultrasound, and for a male, this includes a semen analysis.

2Consultation with a Senior Fertility Nurse

Our senior fertility nurses will take a full medical history and discuss the results of your tests and plan your ICSI cycle for the best chance of success.

3Ovarian Stimulation

The first stage of ICSI is ovarian stimulation. This involves using tailored doses of fertility drugs to help produce mature eggs.

4Egg Maturation

A ‘trigger’ injection of fertility medications is administered to assist with the final maturation of eggs and loosening of the eggs from the follicle walls.

5Egg Collection

Egg collection takes place in our clinic theatre under ultrasound guidance. Before the egg collection, we provide you with a sedative. The eggs are retrieved from the follicles where they have been growing.

6Injection of sperm into mature eggs

The sperm sample is washed and prepared. Our expert embryology team identify and select the best healthy sperm from your sample. We look for active sperm that are moving well and are shaped normally. Each mature egg is then injected with an individual sperm under the microscope. The injected eggs are placed into the time-lapse imaging incubators to monitor for fertilisation. 

7Fertilisation

We monitor your eggs for signs of fertilisation. This usually occurs 16-18 hours after insemination.

8Embryo Culture

The created embryos are placed in an incubator where they grow in the lab for the next two to five days. Our embryologists keep you updated on the process of your embryos and advise when your embryo transfer will take place.

9Embryo Transfer

Our embryologists will select an embryo, which is transferred under ultrasound guidance into your uterus by our fertility consultants. Embryo replacement will take place in our specially-designed transfer room.

10Vitrification

Any remaining good quality embryos that are not transferred are frozen and stored. Frozen embryos can be used in subsequent cycles if the first cycle is not successful or for sibling pregnancy.

11Pregnancy Test

You can take a home pregnancy test around two weeks after your embryo transfer. During this time, you will be supported by our clinic midwife. If your test is positive, you will be booked in for your early pregnancy scan about three weeks later.

 

Risks

As a HFEA-regulated clinic, Manchester Fertility has a duty to highlight any potential risks to patients undergoing fertility treatment. Although fertility treatment such as the ones we provide via Essential Fertility are normally safe, there are some risks to be aware of, such as:

  • Ovarian hyperstimulation syndrome: When ovaries overreact to the fertility drugs needed to produce mature eggs. 
  • Damage to eggs: There are a small number of eggs that are damaged during the ICSI procedure (<5%)
  • Genetic or developmental defects: There have been reports that children born using the ICSI technique are at a slightly higher risk to have genetic problems or developmental defects.  There isn’t conclusive evidence that this is related to the technique but may be linked to the underlying infertility.
  • Multiple pregnancy or birth (twins, triplets or more): If more than one embryo is transferred, there is an increased chance of a multiple pregnancy.
  • Ectopic pregnancy: There is a possibility that during embryo transfer, it could travel to the fallopian tube instead.

Patients should consider these risks, and they will be discussed at your consultation. However, in our experience, most patients who decide to proceed with treatment have no (or few) problems with their health or pregnancy.
